Granny’s Kountry Candles, LLC Fundraiser Application Form
1. My group is committing to sell Granny’s Kountry Candles, LLC line of candles and products
2. All group officials have approved of this fundraiser and we are ready to begin fundraising.
3. We are ready to schedule a start date for our fundraiser and a order turn-in date.
4. When making payment for this fundraiser, I understand that Granny’s Kountry Candles, LLC will only accept MC, Visa, company/organization checks or Paypal. We do NOT accept personal checks. I also understand that in the event a check is returned for any reason, there will be an additional $35.00 fee in addition to the returned check.
5. A minimum of $100 must be sold in order to qualify for “fundraiser status” and receive free shipping. My group will earn 35% or $4.55 on each product sold and this will be deducted from the total that is turned in to Granny’s Kountry Candles, LLC.
6. Any damages or missing items MUST be reported within 3 working (M-F) days to Granny’s Kountry Candles, LLC via email: grannyshelper@metalink.net or phone: 419/923-8700.
7. All orders will be shipped within 10-12 working days from receipt of payment and order.
8. All orders will be verified and confirmed via telephone.
